I consent to allow St Rose Community Church to take and use photos of the child listed on this form to be published online for church purposes only.
● Each leader and students will have their temperature taken upon their arrival. Anyone with a temperature above 100.4 will not be allowed to stay and participate.
● There will be a hand sanitizing station set up and students will be encouraged to sanitize hands upon entering the building and at the end of game time and Bible/Memory Verse time.
● All leaders will be required to wear face coverings. 3rd-12th grade students will be required to wear face coverings. Face coverings will be highly encouraged for the Pre-K-2nd grade students.
● We will not be using the water fountain or providing cups of water. Students can bring a personal water bottle that they must keep up with.
● The furniture and supplies will be sanitized at the end of each meeting.
Unfortunately, we will not be running the bus for Spring of 2021.
If at any point during the semester the school system cancels school or goes entirely virtual, we will cancel Wednesday night activities.
I have read all of the information above and I understand the protocol and procedures and I give permission for my child to participate in the Wednesday Night activities.
